The Coronary Heart Disease (CHD) Risk Assessment Tool generates custom, individualized CHD-Risk assessments for Major Risk Factors, Framingham Point Scores and Metabolic Syndrome. This tool incorporates two main pages. The input page contains input fields for 15 clinical parameters (sequentially numbered), Users' data is entered in this page. The output page presents the Users' CHD Risk Assessment. It is displayed in a 2nd browser window, and contains 2 sections (Users' Profile and Risk assessment). This output page is print-friendly (prints in one letter-size sheet).
